Wegman Short Form
Note: To apply to the John F. Wegman Fund use the Wegman Short Form for requests of $7,500 or less.  Use the Common Application Form with Logic Model and Executive Summary for requests of more than $7,500 or with a total project budget of $15,000 or more.  Applications must be received at least 3 - 6 months before the program start date.

Project Brief Application
The Community Foundation also has a variety of grants available for application that require a letter or a competed Common Application Form.  Please use Search for Funding to determine what is required.

Other Forms
Grant applicants are encouraged to submit photographs of Community Foundation funded activities for possible use in our annual report, newsletters or website.  Please complete and submit the attached Image Use Release Form with your photographs to be considered for use.
