Grant Forms

Because of the diversity of our funding opportunities, there are many different application processes, procedures, deadlines, and forms.  Please do not use the forms below without first checking the requirements for each grant within the Grants search function.  If you are unsure as to which form to use, contact Mary Hartstein at mhartstein@racf.org.

Grant recipients are encouraged to submit photographs of Community Foundation-funded activities for possible use in our annual report, newsletters, or Website.  Please complete the Image Use Release Form and submit along with your photographs to be considered for use.

Application Forms

• Bullis Fund - Community Impact Application 
• Civic Engagement RFP  For reference only
• Common Application form with Logic Model 
• Early Childhood RFP  For reference only
• Wegman Short Form 
  Note: To apply to the John F. Wegman Fund use the Wegman Short Form for requests of $7,500 or less.  Use the Common Application Form  with Logic Model and Executive Summary for requests of more than $7,500 or with a total project budget of $15,000 or more.  Applications must be received at least 3 - 6 months before the program start date.
• Project Brief Application 
  The Community Foundation also has a variety of grants available for application that require a letter or a competed Common Application Form.  Please search on our Grants database to determine what is required.
• Youth & Families RFP  Applications must be postmarked by February 17th, 2012.
